Cafero Wants Answers From Labor Dept. Regarding Tax Form Mistake

House Minority Leader Lawrence Cafero, R-Norwalk, wants to know how the Labor Department allowed 27,000 tax forms go out the door with two different peoples’ names, Social Security numbers, and addresses on each document.

“This is one of the most major violations of a person’s privacy,” Cafero said Monday.

The forms went to 27,000 individuals in 52 cities and towns from Ansonia to Sterling. A total of 250,000 of these forms were sent out to those who claimed unemployment benefits in 2013.

The top portion of the form had the correct information for the individual who was supposed to receive the document. But the bottom portion on each of the 27,000 erroneous forms had information pertaining to separate individuals.

Labor Department Spokeswoman Nancy Steffens said the mistake was caught and corrected immediately. New forms were mailed with the correct tax information.

She said credit monitoring will be provided for anyone who received one of the 27,000 forms.

“We need to know what went wrong and what steps are going to be taken to prevent this from happening again,” Cafero said.

Steffens said it was a coding error.

“To have 10 percent go out that were incorrect boggles the mind,” Cafero said.

It’s not clear if the information was shared with the federal government.

Steffens said the state is doing everything it can to protect those impacted by the error by offering credit monitoring. She said that’s the department’s main focus.

“The mistake was caught and corrected immediately”? No, if that were the case, these forms would never have been mailed out in the first place.
